IMPORTANT NOTE FOR NEW PATIENTS:

You will be emailed a New Patient Intake Questionnaire.

I recommend reserving 20 minutes at minimum to complete the questionnaire. I use comprehensive questionnaires so that I can start our first appointment with context, allowing you to tell your story without interruptions.

After you submit the New Patient Intake Questionnaire, I will review it and you will receive a notification that I have confirmed your appointment.

I review all intake questionnaires within 24 hours.
